The "Busy but Broke" Med Spa Problem

Should I pay commission on gross or net revenue?+

For treatments with high consumable costs (like Botox, filler, or threads), you must pay commission on Net Revenue (Gross Revenue minus Consumables). If you pay on Gross, a promotional discount could actually cause you to lose money on every syringe injected while your provider still gets paid.

What is a standard commission rate for injectors?+

Most med spas pay injectors a base hourly rate (e.g., $40-$60/hr) plus a tiered commission (e.g., 10-20%) on net revenue. Pure 1099 independent contractors might receive a higher percentage (30-40%) but no base pay, benefits, or marketing support.

This is the classic "busy but broke" med spa problem. It usually stems from paying too high a commission on Gross Revenue for injectables. The provider makes great money, but after paying for the product, rent, marketing, and the provider's commission, the clinic actually takes a loss.
