How to calculate CAC and LTV properly
Customer Acquisition Cost (CAC):
CAC = (Total marketing spend + management fees + ad spend) ÷ New patients acquired that period
Include everything: agency fees, ad spend, tools, content production, review platforms, any dedicated marketing salary. Exclude patient care staff, rent, medical supplies.
Lifetime Value (LTV):
24-month LTV = Average first treatment value + (Average repeat treatments per year × 2) × Average treatment value + Membership revenue if applicable
Most Canadian clinics use 24-month LTV because it’s long enough to capture repeat behaviour but short enough for realistic modelling.

The LTV:CAC ratio that predicts growth
The single most important number in aesthetic clinic marketing:
- LTV:CAC of 1:1 or below: unprofitable — marketing is breaking even or losing money
- 2:1: profitable but too tight to scale
- 3:1: healthy, scalable
- 5:1: excellent, aggressive scaling justified
- 10:1: exceptional, usually indicates untapped scaling opportunity
Most healthy Canadian med spas run at 3-5:1. Cosmetic surgery clinics often hit 8-15:1 given procedure values. If your clinic is below 2:1, marketing needs fixing before scaling — typically the fix is on the retention/LTV side, not the acquisition/CAC side.

- Booking software with source attribution (Mindbody, Vagaro, Boulevard)
- Google Ads conversion tracking with offline conversion imports
- Meta Pixel + Conversions API
- GA4 with proper conversion events
- Call tracking (CallRail or similar) for phone bookings
- Monthly reconciliation of booking source vs marketing channel

When to scale marketing spend
Scale spend when all four of these are true:
- LTV:CAC ratio is 3:1 or better
- CAC has stabilised for 60+ days (not still rising)
- Booking calendar utilization is 75%+
- You have capacity to add staff or hours to absorb new bookings
